Usage and dosage for HIV-1 treatment: The dose is 300 mg (1 tablet) each time, once a day, orally, on an empty stomach or with food. Dose adjustment for adults with renal impairment When tenofovir disoproxil fumarate is administered to subjects with moderate to severe renal impairment, drug exposure is significantly increased (see [Pharmacokinetics]). For patients with a baseline creatinine clearance rate of <50 mL/min, the administration interval of tenofovir disoproxil fumarate should be adjusted as follows. The recommended dosing interval here is based on a single-dose pharmacokinetic data model in non-HIV and non-HBV infected subjects with different levels of renal impairment, including patients with end-stage renal disease requiring hemodialysis . In patients with moderate to severe renal impairment, the safety and efficacy of these dosing interval adjustment recommendations have not been clinically evaluated, so the clinical response to treatment and renal function should be closely monitored in these patients (see [Note matter]). For patients with mild renal impairment (creatinine clearance 50-80 mL/min), there is no need to adjust the dose. In these patients, the calculated creatinine clearance and serum phosphorus should be monitored regularly (see [Precautions]). Dose adjustment for patients with changes in creatinine clearance (recommended 300 mg dosing interval) Creatinine clearance is calculated using ideal (lean) body weight. Creatinine clearance 50 mL/min: once every 24 hours; creatinine clearance 30-49 mL/min: once every 48 hours; creatinine clearance 10-29 mL/min: once every 72-96 hours; hemodialysis Patients: Once every 7 days or after about 12 hours of total dialysis (usually once a week (assuming 3 times a week for hemodialysis, each lasting approximately 4 hours). Tenofovir disoproxil fumarate should be administered after completion of dialysis] In non-hemodialysis patients with creatinine clearance <10 mL/min, the pharmacokinetics of tenofovir has not been evaluated, so There are no dosing recommendations for these patients. There are no data on dosing recommendations for children with renal impairment.
